Team Description
A small team based in Central Bradford working in some of the communities with poorest health outcomes across Bradford. Working closely with many partners at all levels, looking at an organic, sustainable and whole systems approach to improve health through active travel. Lots to do, never a dull moment and always fun, with lots of different skills we are a team that works together to maximise opportunities and get things done! As a test and learn project we don't expect you to be an expert in everything, just open minded and ready for a challenge.
Role Description
We are looking for an Active Travel Social Prescribing Officer to join our dedicated team in the Department of Place. This role is an opportunity to make a real difference in our vibrant and diverse city.
The main responsibilities of this role:
- Maintain strong and on-going relationships with the NHS, Bradford District and Craven Health and Care Partnership, Primary Care Networks (PCNs), Social Prescribers, Community Partnerships, Voluntary, Community and Faith Settings (VCFS) organisations, delivery partners, stakeholders and communities in support of the prescription of walking and cycling for utility trips otherwise known as Active Travel to increase take up within the pilot areas in order to improve wellbeing and reduce health inequalities.
- To support the Active Travel Social Prescribing Manager to successfully implement the development and delivery of programmes and projects included within the Active Travel Social Prescribing Programme.
- Engage with communities and stakeholders to promote programme of work in relation to the Active Travel Social Prescribing Pilot Study, facilitating/attending events where appropriate.
- Contribute to the necessary commissioning and procurement of goods and services in connection with the pilot study and operate a small grants process.
- Support investigations appropriate to area of expertise including where appropriate with other public bodies.
