Team Description
- The East Social Work Team who works with older people within the Department of Adult Social Care are looking to recruit to a full time Advanced Practitioner post.
- The team consists of ten qualified social workers, four Community Care Officers and a Care Navigator.
- If you are passionate about social work and are confident in upholding and promoting the rights of people who use the service, we want you to be part of our team.
- You must hold a social work qualification and must be registered with Social Work England.
- Applicants will have a sound understanding of the Care Act 2014, Mental Capacity Act 2005 and be able to demonstrate how they apply this in their practice. Candidates will have good working knowledge of the National Framework for NHS Continuing Healthcare.
- We want you to be part of our team so that you can undertake partnership work to support adults and carers in decision making through collaboration with others within the service and external partners.
Role Description
e.g. We are looking for a Advanced Practitioner to join our dedicated East team in the Adult Social Care & Health Department. This role is an opportunity to make a real difference in our vibrant and diverse city.
The main responsibilities of this role:
- Supporting management team in providing a high-quality social work experience in Adult Services assisting the Team Manager in support and development of staff in the team.
- Mentor and support new social workers, including support and supervision responsibilities to newly qualified social workers undertaking the ASYE to support their learning, seek out opportunities for the development and coaching as required.
- Contribute to practice development, undertaking the lead to links to Adult Services with Early Help and Prevention and Community led support work and neighborhood wardens
- Excellent communication, facilitation, and collaboration skills.
- Strong knowledge of social work practice, statutory requirements, and case recording standards.
- An Experienced Social Worker with experience using case management systems.
