Community Care Officer

Job Introduction

We are seeking a passionate and experienced Community Care Officer to join our dedicated team in the Shipley Older People Assessment and Support Team. This is a unique opportunity to make a meaningful difference in the lives of older adults in our vibrant and diverse city.

As a Community Care Officer, you will demonstrate a thorough understanding of adult services, with a particular focus on safeguarding and promoting the welfare of older peopleYou will be working with integrated multidisciplinary teams alongside health colleagues and qualified Social Workers. You will bring experience in working collaboratively with adults and their carers, assessing their needs, and developing effective, person-centred support plans. Your role will also emphasise key social care priorities, including the implementation of the Care Act 2014, safeguarding principles, and strength-based practice

 You will need to:

  • Understand and respond to harm or abuse – Recognise the signs and respond proportionately.
  • Assess risk and build emotional resilience – Reflect on situations and make informed, balanced judgments.
  • Act within procedures and partnerships – Work in line with local policies and collaborate with internal and external partners.
  • Communicate effectively – Share clear, succinct, and objective information to aid joint decision-making.
  • Be proactive and solution-focused – Be prepared to challenge and be challenged.
  • Understand legal responsibilities – Apply the Care Act, Mental Capacity Act, and safeguarding legislation while respecting others’ roles and joint procedures.

You will need to demonstrate the ability to put person-centred values into practice, promoting the dignity, rights, and individuality of the people you support, while helping them plan for their future well-being and fulfilment. To succeed in this role, you will need enthusiasm, adaptability, and a positive attitude toward change.

Experience in adult services is essential. Experience in assessment and care planning is essential Knowledge of the Care Act 2014 and Mental Capacity Act are also essential to this post.  

This post requires an enhanced disclosure check with the Disclosure and Barring Service (DBS).
