Community Care Officer

Job Introduction

We are looking for a caring and compassionate Community Care Officer to join our Assertive Outreach Team. In this role, you will work with individuals who have a primary diagnosis of psychosis and face significant challenges in their daily lives. Many experience unstable living situations, such as homelessness, severe substance misuse, and difficulty engaging with services. Our clients may also struggle with daily living skills and be at high risk of self-neglect, have a history of involvement with the criminal justice system, or frequently require inpatient psychiatric services.

Our mission is to provide support, understanding, and empowerment to help our service users navigate life’s complexities. We are seeking an individual who is eager to work with people who have complex mental health needs and are difficult to engage. The ideal candidate will be able to work creatively, flexibly, and effectively in uncertain environments, developing meaningful working relationships to identify and understand the needs and strengths of each individual.

We are looking for someone who thrives on small wins, understands people holistically in a non-judgemental way, and is not fazed by less-than-polite rejection, maintaining a politely persistent approach.
