Position Title: Reablement Assistant (Days)

Band/Grade: 5

Hourly Rate: £13.26ph - £13.47ph

Contract Length: Permanent

Working Hours:  2 posts:

                                    24.75 hours per week

                                    24.25 hours per week

                                    

Work Location:  Eden Gardens/Wagtail Close Extra Care Setting, Bradford 6.

There are 33 properties within Eden Gardens/Wagtail Close where individuals who require varying levels of support to maintain their independence and daily living are supported by the staff team.

Our Department of Adult Social Care at Bradford Council has an exciting opportunity for a Reablement Assistant

Our team is looking for flexible and multifaceted individuals who can work within an Extra Care setting team. 

Let’s talk about your skills and experience….

  • To work with a diverse service user group, predominantly with adults aged 35 and over with differing disabilities, mental health, dementia and complex needs.
  • Support service users with their individual needs and daily living skills, encouraging independence where appropriate, supporting and maintaining happy healthy lives for those accessing services.
  • Greeting, assisting, and directing visitors who will be visiting.
  • Ensure medication is administered in accordance with the medication policy.
  • Complete clear and concise daily reports and support plans. 
  • QAF Diploma (NVQ/QCF) at (minimum) Level 2 in Health & Social Care.
