Job Introduction
The Continuing Health Care Social Work Team within the Department of Adult Social Care is looking to recruit two full-time social workers. You must hold a social work qualification and must be registered with Social Work England. If you are passionate about social work and are confident in upholding and promoting the rights of people who use the service, we want you to be part of our team. Applicants will have a sound understanding of the Mental Capacity Act 2005 and be able to demonstrate how they apply this in their practice. Candidates will have good working knowledge and experience of the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care.
We want you to be part of our team so that you can undertake partnership work to support adults and carers in decision making through collaboration with others within the service and external partners. The successful candidate will demonstrate the ability to communicate effectively, share appropriate, succinct, objective information and analysis to aid in joint decision-making with our health colleagues. You will be proactive, persistent, and willing to challenge and be challenged. You will effectively apply joint procedures and know your responsibilities and those of others. The successful candidate will:
- Have experience in relevant settings after completing a degree in Social Work.
- CQSW/CSS/DIPSW BA/MA in Social Work
- Be registered with Social Work England.
- Have good working knowledge of the National Framework for NHS Continuing Healthcare and NHS funded Nursing Care.
- Have knowledge of relevant health and social care legislation, including the Care Act 2014.
- Hold a full UK Driving licence, unless a disability prevents this.
