The Team
The team, work with young people and adults with learning and physical disabilities including those who are neurodiverse or autistic, their families, and other organisations to uphold their rights, build on their strengths and ensure they remain connected to what is important and local to them. You will be based in a Locality Adults with Disabilities Team holding a case load allocated to you by your Manager.
The aim is to support people to remain in control of their lives, and live at home with people they love, remaining as independent as possible. This involves safeguarding their rights, respecting their autonomy and using the flexibilities within the Care Act and Care and Support statutory guidance to enable people to self-direct their support.
The role and the main responsibilities
Role Description
We have an exciting opportunity for you to join our forward thinking, rights based Adults with Disabilities service. We have a team of skilled, dedicated, friendly and supportive staff who are committed to upholding people’s human rights through their social work practice. However, as our service evolves, we are looking for a new Social Worker colleague to join us and continue to develop and provide a high quality service for the people we support.
You will have a strong commitment to human rights at the heart of social work practice and will know and understand the importance of the UN Convention on the Rights of the Child and the UN Convention on the Rights of Persons with Disabilities. You will not want to make any decision without the person you are there to support being in control and being enabled to fully participate.
The main responsibilities of this role:
- Contribute to the provision of Social Work support for Adults with Disabilities in the community and other settings.
Provide an effective Social Work service including assessment, support planning and reviews.
Safeguard Adults including those at significant risk of harm, upholding their rights, assessing and managing risk and undertaking critical decision making including taking professional responsibility for decision making.
Operate within departmental policy, practice and procedures.
