Care Home Brokerage Officer

Job Introduction

The Support Options Team is responsible for finding appropriate community support for people who want to live independently in their own home or have been assessed with a need to move into a residential and nursing placement.

We are looking for Care Home Brokerage Officers to join our new dedicated team in the Department of Adult Social Care. This role is an opportunity to make a real difference in our vibrant and diverse city. 

 The main responsibilities of this role:

  • To broker external residential and nursing care home provision for adults with an assessed care need.  
  • To liaise with people planning to move to residential or nursing care home provision, their families and carers to understand their needs, preferences and financial contribution to third part top-ups in order to identify the most suitable care home options for them.
  • To support processing, set-up and oversight of care home placements which includes regular liaison with colleagues in Financial Support Services, Social Work Teams, Commissioning and Contracts teams and External Providers. 

An enhanced disclosure check with the Disclosure and Barring Service will be undertaken for this post.

Please note the successful candidates will not commence the posts until November 2025. This is due to the Care Home Brokerage Team being newly formed which will involve further recruitment and training.
